Cryopreservation, Storage & Shipment
Cryopreservation and analysis are included in the cost of SPAR.
Patients may consent to have the Foundation store their specimens indefinitely or ship them to an infertility clinic.
Storage
SPAR fees include nine months of specimen storage. Afterward, storage costs $50 per month. Patients renew sperm storage every two years by signing a form mailed to them. Three attempts to contact you will be made before specimens are disposed.
I have not received my renewal form:
Shipment
Cryopreserved specimens may be shipped worldwide. Patients need to sign a consent form and pay outstanding lab fees in advance of shipping.
Scheduling
In order to protect the specimens, we ship between the hours of 9 AM and 12 PM EST on Monday, Tuesday, and Wednesday.
Liquid Nitrogen Dry Shipper
Keeps the sperm at liquid nitrogen temperatures for shipping.
IMPORTANT
Specimen Shipment is a regulated process. Patients must return a signed “consent to ship” form and pay all outstanding lab fees at least two weeks prior to shipment.
Once there are cryopreserved sperm from at least two specimens with undetectable viral loads, the Program Coordinator will send a specimen shipping notification form to complete. This will allow us to create a consent form for shipping the patient’s specimen.
I’m ready to schedule my shipment:
